Pancreatic enzyme replacements come in capsule form. Inside each capsule are many small "beads" that contain digestive enzymes. Each bead is covered with a special "enteric coating." This coating allows the beads to dissolve in the small intestine. The digestive enzymes are then released in the small intestine to help digest food. The main functions of enzymes are: to digest carbohydrate, protein and fat the three parts of food that supply calories to help with weight gain; and to promote nutrient absorption. More than 90% of people who have CF take pancreatic enzyme replacements.

In this addition to the APCELL series of experiments, McNaughton describes an experiment for identification of pharmaceutical drugs such as aspirin, salicylic acid and paracetamol by IR and Raman spectroscopy in conjunction with a searchable database. In order to improve the students' theoretical understanding of these techniques, the measurements are preceded by use of computer simulation software that animates molecular vibrations for a range of small molecules. Students learn to visualise the molecular vibrational motions, and relate these to the spectra observed. Continuing the international contributions to this Journal, Dhinsda in Brunei reports on a research study into the understandings of pre-service science teachers' conceptions of the acid-base-related concepts pH, acid, base and neutral. Very limited, narrow and `textbooky' understandings were found to be common. There are enough research studies on understandings of these concepts by students in a range of levels in other parts of the world to suggest that these findings cannot be dismissed as peculiar to Brunei, or to the competence of its teachers - any more than in other countries. These days there is considerable focus on learning generic, transferable skills within the context of courses in science. Downstream signaling of ER, it has been hypothesized that administration of only one drug may not be sufficient to achieve complete estrogen blockade. A preclinical nude mouse model with MCF-7 p450Arom up-regulated tumor cell implants has shown that whereas both the AI letrozole and the ER down-regulator fulvestrant slow tumor growth considerably, a combination of the two causes considerably more growth inhibition 45% ; and remained free of growth for the duration of the experiment 29 weeks; ref. 28 ; . Thus, achieving a more complete estrogen blockade may delay the development of hormone-independent signaling pathways that regulate proliferation. This concept of complete estrogen blockade was the rationale behind the combination arm of AI arimidex ; + SERM tamoxifen ; in the adjuvant Arimidex, Tamoxifen, Alone or in Combination trial 3 ; . The subsequent finding that the AI + SERM combination did no better than SERM alone and was, in fact, inferior to AI alone, considerably dampened enthusiasm for attempting complete estrogen blockade. Reasons why this result may apply to an AI specifically in combination with tamoxifen have been offered. Some have hypothesized that tamoxifen's estrogen agonist effect may be amplified in the presence of ultra-low estradiol levels. More recently, the efficacy of a different combination AI atemestane ; + SERM toremifene ; was attempted in the hope of exploiting the weaker estrogen agonist effect of toremifene in a low estrogen environment. Of note, the combination was found to be equivalent to an AI letrozole ; alone among women with metastatic breast cancer 29 ; . This finding suggests that the concept of complete estrogen blockade should be pursued with novel, less ER-agonistic SERMs in combination with AIs. Thus, a North American Breast Intergroup Southwest Oncology Group Cancer Trials Support Unit trial of anastrozole + fulvestrant versus fulvestrant alone in women with metastatic breast cancer is currently enrolling patients. Amgen's increasing efforts to enter the rapidly growing diabetes market and the potentially lucrative obesity market highlights a shift in Amgen's strategy to embrace primary care markets. The acquisition of Tularik will provide Amgen with the tools to further diversify its developmental focus towards small molecules, which have particular application in the primary care setting. This is a higher risk strategy, given the competitive pressure in primary care markets. In contrast, there are significant barriers to entry in Amgen's traditional area of strength, the biologicals market, including the requirement for specialised manufacturing capabilities, the reduced potential for market entry of "me-toos" and the under-developed nature of the biogenerics market compared to traditional generics. This is especially true when the study under consideration reports multiple between-group statistical comparisons, because multiple comparisons markedly inflate the actual Type I error rate and require a much more stringent statistical adjustment.11-15 There is also concern about the Type II error rate of clinical trials i.e., the so-called error ; , which reflects the likelihood of concluding incorrectly that a useful treatment is of no value.16 For example, one recent trial17 found that after 2 years of treatment, sustained disability progression was nonsignificantly reduced by 12%. Clearly, such a result cannot be used to reject a true 12% reduction in this measure, and, in fact, this nonsignificant observation is still compatible with an even more robust treatment effect.16 The issue is the statistical power i.e., 1- ; of the clinical trial to detect group differences and this, in turn, is related to the number of subjects studied.16 In this particular trial, 17 the number of subjects studied i.e., 251 ; provided insufficient power to detect a 12% change on this outcome. If a much larger number of subjects had been entered into the trial, and if the same magnitude and variability of the treatment effect had been obtained, this change would have been statistically significant. As a consequence of such difficulties, it is important to recognize that negative results from small clinical trials generally provide little assurance that a true treatment effect has not been missed. Second, because it is uncertain which outcome measures correlate best with future function, clinical trials that use a combination of outcome measures, including both clinical and confirmatory MRI measures, should be judged as stronger evidence than those that rely on only a single measure, especially when that measure is a subjective clinical score. Therapy was administered in female patients with localised forms of mastopathy. Apart from the standard examination, cytological needle biopsy - in cases with cysts ; and histological punch biopsy - in cases with local fibroadenomatosis ; identification and histological immunochemical investigation ER, PR, her 2 neu ; were made. Toremifene in the doses of 20 mg daily in combination with progestin in the latter half of the cycle was administered in the experimental group for 6 months. Standard phytotherapy was administered in the control group. Patients of both groups noticed alleviation of pain syndrome in 79 % and 50 % of cases correspondingly; 80 % of patients from the experimental group and 60 % - of the control group stopped complaining of pre-menstrual syndrome; disappearance of palpable indurations was noticed in 85 % of patients from the experimental group versus 56 % of patients from the control group; and positive results of mammography and USI were found in 78.4 % of patients from the experimental group versus 25 % of patients from the control group. Diagnosis of local forms of mastopathy by using punch biopsy followed with immunochemical histological investigation and needle biopsy makes it possible to decide on the treatment method surgical or conservative ; . High efficacy and good tolerance of Toremifene in combination with progestins make it possible to use such scheme in the treatment of mastopathy in women with active menstrual and ovarian function.

COMECON was revived and sprouted a number of working committees, but trade remained mostly bilateral. We shall deal with each in turn. The Jalta Agreement had placed Poland in the Soviet sphere of influence. Along with the recuperation from heavy war damage and human losses, which had not favoured scientific activities, the period from 1945 to 1956 saw the implantation of the Soviet-type economic system in Poland. In the immediate postwar years, Soviet rulers concentrated on gaining full political power and eliminating opposition, economic reconstruction, nationalisation of industry, and agrarian reform, while work proceeded on the first three-year plan for 1947 1949. The principal target were political forces close to the Polish government in exile, and the aim to eliminate the influence of social democrats. The so called `battle for the trade', waged in periodicals and action in 1947, was aimed at absorbing trade into the statedominated structures and strengthening the communist Polish Workers' Party, and was directed both against private trade and cooperatives dominated by old-time socialists. It took the form of an attack on the Central Planning Office, which was being accused of using an accounting method which underestimated the share of the working class in national income creation. Authorised publications and official announcements supplied an all-embracing pressure demanding the rejection of earlier Polish non-communist economic thought as well as non-Marxist Western economics. At the first Congress of Polish Science in 1951 it was declared that Polish science had to be rebuilt from scratch according to the Soviet pattern, and that the "fundaments of economic science are lying in Marxist political economy which is the only true science of economic relations among people". Partisanship was not to be feared.
