Denver Boulder Formulary The following is an alphabetical listing of the drugs on the Kaiser Permanente formulary. This list is arranged alphabetically by generic name. Kaiser Permanente has numerous brand name medications on the formulary. In many cases, a generic equivalent is used. Section Heterotrophic Plate Count HPC ; formerly known as the standard plate count, is a procedure for estimating the number of live heterotrophic bacteria and measuring changes during water treatment and distribution in water or in swimming pools. Colonies may arise from pairs, chains, clusters, or single cells, all of which are included in the term "colony-forming units" CFU ; . Method: There are three methods for standard plate count: 1. Pour Plate Method The colonies produced are relatively small and compact, showing less tendency to encroach on each other than those produced by surface growth. On the other hand, submerged colonies often are slower growing and are difficult to transfer. 2. Spread Plate Method All colonies are on the agar surface where they can be distinguished readily from particles and bubbles. Colonies can be transferred quickly, and colony morphology easily can be discerned and compared to published descriptions. 3. Membrane Filter Method This method permits testing large volumes of low-turbidity water and is the method of choice for low-count waters. Material: i ; Apparatus Glass rod Erlenmeyer flask Graduated Cylinder Pipette Petri dish Incubator ii ; Reagent and sample Reagent-grade water Nutrient agar Sample Procedure * 1.Boil mixture of nutrient agar and nutrient broth for 15 minutes, then cool for about 20 minutes. 2.Pour approximately 15 ml of medium in each Petri dish, let medium solidify. 3.Pipet 0.1 ml of each dilution onto surface of pre-dried plate, starting with the highest dilution. 4.Distribute inoculum over surface of the medium using a sterile bent glass rod. 5.Incubate plates at 35oC for 48h.

The genus Peniophora Cooke lacks an amyloid reaction entirely while other genera, e.g., Scytinostroma Donk and Albatrellus S.F. Gray, contain both amyloid and non-amyloid species. Most species in the russuloid clade have basidiospores with an ornamented surface. The nature of this ornamentation has been extensively studied e.g., Capellano and Keller 1978, Keller 1986, 1997 ; . An outer layer, tectum, forms the ornamentation, and the same layer is responsible for the amyloid reaction. The tectum can be covered by up to three additional layers. Species with smooth basidiospores lack a tectum, and in TEM no difference in spore wall structure between amyloid and non-amyloid spores can be detected Keller 1997 ; . We invariably have found the presence or absence of an ornamentation to be a generic character within the russuloid.
